Community Health Alliance, through Sojourner Recovery Services and Transitional Living, provides comprehensive health and social services that empower individuals, families, and communities. We believe in innovation, integration, and collaboration to deliver personalized support across mental health, addiction recovery, housing, employment, and crisis intervention. Our services include case management to help individuals develop essential life skills, counseling and therapy offered in-person and via telehealth, and 24/7 crisis services through the 988 Suicide and Crisis Lifeline. We provide medical services, including addiction medicine, primary care, and psychiatry, to enhance quality of life. Our residential treatment programs offer safe and structured environments for recovery, while housing support helps individuals work toward independent living with a strong safety net. We collaborate with local agencies to promote employment success, offer peer support to encourage wellness, and provide outreach for individuals navigating the criminal justice system or experiencing homelessness. Through forensic and perinatal services, we assist incarcerated individuals with mental health needs and support pregnant women and mothers facing substance use challenges. By meeting people where they are and providing the resources they need, we help build stronger, healthier communities.
